Successful applicants will have:
Florida RN/LPN Licenses
Level II Security Check ACHA
One year minimum of Home Health experience
Current CPR Card
Physical within six months (stating free of communicable disease)
PPD or chest x-ray
Two professional references
Two government issued I.D's (one must have a picture)
